National Case Management Week


WHEREAS, the Case Management Society of America (CMSA) is responsible for developing evidence-based tools and resources to help ensure better patient outcomes; and

WHEREAS, case managers work in a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services necessary to meet an individual’s and family’s comprehensive health needs; and

WHEREAS, case management plays a critical role in ensuring the proper use of healthcare dollars, while maintaining a primary and consistent focus on patient healthcare outcomes; and

WHEREAS, Case Management Week recognizes the contributions and commitments case managers make to improving healthcare for Virginians and other citizens of the United States;

NOW, THEREFORE, I, Terence R. McAuliffe, do hereby recognize October 11-17, 2015, as NATIONAL CASE MANAGEMENT WEEK in our COMMONWEALTH OF VIRGINIA, and I call this observance to the attention of all our citizens.
